Please read the following and sign below to signify your agreement to the following policies.
GENERAL TERMS
I hereby release, acquit, hold harmless and forever discharge Church at The Mill, its agents, servants and employees and all persons, natural or corporate, in privity with them or any of them, from any and all claims or causes of action of any kind whatsoever, including but not limited to actions, suits and/or claims for any bodily injuries, death or property damage which may be sustained by me (named below) while participating in Creative Arts Camp June 3-7, 2024, including any prep work ahead of camp week or clean up afterward, travel to and from such activities, not resulting from the intentional tortious act or acts of any agent, servant or employee of Church at The Mill. It is acknowledged that the decision to engage in the above activities is entered into freely and that Church at The Mill, its agents, servants, and employees have not influenced the decision to engage in such activities. I also understand that Church at The Mill is a Christian organization and that I may be exposed to Christian principles and Biblical activities.
I understand that, in the event medical intervention is needed, every attempt will be made to immediately contact the emergency contact listed on this form. In the event we cannot reach your emergency contact, I hereby give my permission to the physician or dentist selected by the activity leader to hospitalize, secure medical treatment and/or to order an injection, anesthesia, or surgery as deemed necessary. I understand that my insurance coverage will be used as primary coverage in the event medical intervention is needed.
I understand that Creative Arts Camp participants and volunteers may be video recorded and/or photographed. I hereby authorize Church at The Mill and/or their agents to use photographs or videos of me for promotional purposes. I specifically understand that CATM shall hereby retain any and all rights in respect to the photograph(s) and/or video/film project(s)and/or website/internet project(s), including but not limited to, the rights to reproduce, copy/edit, exhibit, publish, or distribute such photograph(s) and/or video/film project(s) and/or website/internet project(s).
LIFESTYLE COVENANT
As a volunteer leader, know that you are in a leadership position and will be a role model for children and students. As such, it is important that you represent well the values and beliefs of Church at The Mill. Important lifestyle issues include:
Relationship/Sexual Behavior
We hold a biblical view of marriage. We believe that marriage was designed by God to be a committed, monogamous relationship between a man and a woman. We believe that sex was created by God as an expression of intimacy within the context of biblical marriage. Volunteers who embrace a lifestyle that contradicts these beliefs will find themselves having to pretend to be something they’re not and believe something they don’t.
We may ask you not to serve for the following reasons:
- If you are involved in a romantically intimate relationship with someone of the same sex.
- If you are involved in a sexual relationship and not married.
- If you are currently living with someone of the opposite sex and not married.
- If you are married and are currently involved in a romantically intimate relationship outside of your marriage.
Substance Behavior
If you have a history of alcohol or drug abuse, this might be something God uses in a positive way to impact the future of the next generation. We’d love to talk with you further about your story and how it can impact the children and students in our ministry.
We have a few guidelines that will determine your volunteer placement, and a few reasons we may ask you not to serve at this time:
- If you have been arrested for or convicted of an alcohol or drug-related offense in the past 12 months.
- If you are currently being treated for alcohol or drug abuse.
- If you have used any illegal substance in the past twelve months.
- If you consume alcohol to the point of being unable to drive legally
Online Behavior
The students (and/or the parents of the students) in your group will definitely visit your social media platforms. We think social media can be a great way to connect with families outside of programming! However, they’ll also be able to see everything you post publicly. Please leverage your social media in a positive way. Please be mindful of everything you post.
We may ask you not to serve for the following reasons:
- If you use inappropriate language.
- If you post inappropriate pictures.
- You promote anything that does not represent the values of CATM.
- Any inappropriate communication with a child or student.
By signing here, I agree to the terms above as long as I am an Adult Leader at CATM. If I am unable to uphold this covenant I understand that I will need to resign from my volunteer position or that I may be asked by the leadership to resign.
